Recovery and Rehabilitation Our homecare services have the primary clinical purpose of aiding recovery and rehabilitation of a client with an impairment, activity limitation or participation restriction owing to a long-term illness or injury.

CLCA is able to deliver safe and effective care packages through high-quality and well-led nurses and carers. We provide this at home, which directly supports the Discharge to Assess (D2A) pathway run by NHS England, which has the aim of improving discharge times and in-turn freeing up hospital beds. Allowing our clients to preserve a beneficial level of independence in tandem with a tailored care plan, places them in the best environment to work towards a successful rehabilitation or recovery.

Our staff receive on-going training and supervision to safeguard the rigorous requirements that our clients expect and deserve.

Complex Care Services Having complex health conditions does not mean that your quality of life should be affected. At CLCA, we tailor care packages to ensure that our clients have maximum independence and to also offer relief and support to family members.

Our dedicated team of highly qualified nurses and health care assistants have the expertise to allow clients to remain in their own home and still receive the same standard of care that they would have in a hospital or care home environment. Our staff receive on-going training and supervision to safeguard the rigorous requirements that our clients expect and deserve.

As well as providing clients with new care packages, CLCA specialises in managing failing care services. Our team can respond quickly and effectively to prevent re-admission to institutions, which can often have a detrimental effect on physical and mental conditions. We understand that when clients feel comfortable and supported, they will respond to care and treatment in a much more positive manner. This is why we endeavour to make sure our homecare services accommodate even the most complex of health conditions.

We can provide care plans for clients requiring recovery and rehabilitation support as they move from hospital to home. CLCA supports the Discharge to Assess (D2A) pathway run by NHS England and can assist with clients suffering from:

- Strokes.
- Head injuries.
- Acute/Traumatic brain injuries.
- Spinal Cord injuries.
- Degenerative Spinal diseases.
- Long-term mental health illnesses including Dementia and Alzheimer's.

Palliative Care We provide holistic care to clients with advanced progressive illnesses for as long as they need it in the comfort of their own home. Regardless of this timeframe, we are committed to helping our client's live as well as possible and to reach the end of their life with dignity.

Having a loved one in this position we understand you want the best care and services given to them. It is a very personal matter, affecting people in different ways, which is why our clinical manager really takes the time to understand and assess our clients' condition to ensure the care package we create is tailored to their needs. This does not only involve the management of their condition, but our team of experts in palliative care will provide the reassurance and comfort for them and their family right to the end. They treat people with compassion so you can be assured your loved one and the entire family is receiving the kind of care they deserve and utmost support through this difficult time.

Our goal is to help prolong the lives of our clients' and to promote their independence wherever feasible. Whether it is general care or specialist care and 24 hours, live-in or part-time, we can assure you our nurses will be there as required and can react immediately to your needs.

Respite & Holiday Care No matter how committed you are to the person you are caring for, sometimes people just need a break. This should never be seen as a weakness or a failure. It is entirely natural and at CLCA, we fully appreciate the hard-work and dedication needed in caring for someone. We are always on-hand to support you with short-term or emergency care with our dedicated team of nurses and carers.

Furthermore, as well as respite care, we are also able to offer our services to help when taking the person requiring care on holiday. We understand how important quality of life is and being able to go away on holiday can contribute to this. Our staff will help with the potential strain and stress this can cause by providing expert care aligned with an existing care plan. CLCA can also produce a care package specific to the break/holiday to help safeguard against issues arising because of complex health conditions.
